The MIDAS questionnaire is validated and self-administered — your neurologist will accept it. Bring the score and the date you filled it out to your appointment. Most insurers in the US require a documented MIDAS or HIT-6 above a threshold before approving CGRP inhibitors (Aimovig, Emgality, Ajovy, Vyepti).
